Hi Bartman, lay the hook alongside the plastic and mark the plastic on the back where the hook should come out (where the hook bends) you put the hook point in the nose of the softbait, then push it in, feeding it up the hook and allow the hook to come out the back (top) and then push the head part firmly over the keeper at the base of the hook. The keeper is the barb, or ridges of lead that stop it sliding off again. It should lie straight on the hook, with the nose of the plastic snug up against the lead head, and you are ready to fish.
