Is this the correct form to import fish from Namibia to South Africa ? I will travel via Botswana

Hi. I and a few of my friends travelling independently have never used this form . We go to Henties most years. We follow the fishing permit regulations of 10 fish maximum per person , fish must be size and in the case of Kabeljou only 2 over 700 or 800 mm. I think a max of 50 fish per car also applies , not sure as we travel 3/4 per vehicle. Never had a hassle except for people asking for a fish or two at borders.  .
